Self-registration for children

• Register on Cowin.gov.in website using your mobile number and OTP. Select CK Birla Hospital, Gurgaon 122018 or CK Birla Hospital, New Delhi 110026 as vaccination centre (Upto 4 appointments can be scheduled with one mobile number) [Strictly online booking, no walk-ins]

Self-registration for adults

• Option1: Register on Cowin.gov.in website using your mobile number and OTP. Select CK Birla Hospital, Gurgaon 122018 or CK Birla Hospital, New Delhi 110026 as vaccination centre (Upto 4 appointments can be scheduled with one mobile number)
• Option 2: Walk-in directly to the hospital with mandatory documents*
• Option 3: Register via Aarogya Setu app

Eligibility

• Age 15+ years

Note: Children aged between 15-18 years are eligible to receive only COVAXIN

• Aadhar card/ Valid school ID card for children
• Government authorised identity proof (Aadhaar /PAN /Voter Id) is required for adults
